Annual Membership 

Ages 12 years - 
59 years

$ 1,500 annually

Ages 60 years - 69 years

$1,800 annually

Ages 70 years
and up

$2,100 annually

We do not contract with insurance companies. You are responsible for the cost of membership without regard to insurance coverage. We do not accept insurance and you will not be able to file an insurance claim for your visits. If your insurance plan has an HSA or FSA plan, you CAN use it as a form of payment because we are classified as a medical clinic with a licensed physician.


You CAN use your health insurance for services outside our office like seeing your other health care providers, labs, imaging costs and other health care needs.

Direct Patient Care membership costs may be lower than your deductibles. Health care costs continue to rise and deductibles for some people have skyrocketed. It is not uncommon to see deductibles in excess of $5,000 or higher. So, whether you go to a clinic that accepts insurance, or you come to one like ours, your out-of-pocket expenses may be comparable to your deductible. 


Medicare Opt-Out. We have chosen to opt-out of the Medicare system. Therefore, services with Fort Worth Direct Care are NOT eligible to be covered by Medicare. You will NOT be able to file an insurance claim for your membership cost. All Medicare recipients are required to sign a contract with our office that acknowledges Dr. McFadden has Opted-Out.
